Evidence
Evidence in PortfoliOH is a submission that shows what you’ve done and what skills you’ve developed. A verified evaluator—such as a teacher or external reviewer—will review and assess your submission.
Sources of Evidence
You can add any evidence that demonstrates specific skills. Your evidence can come from a variety of experiences, including:
- Work-Based Learning
- Community Service
- School Projects
- Other meaningful activities in your community
Just remember: nothing is verified until a teacher, mentor, or supervisor confirms that you demonstrated the documented skill(s).
Capturing Evidence
Every evidence submission in PortfoliOH includes a written reflection where you explain what you did and how it showed your skills.
You may also be asked to include:
- A link to something you created or worked on
- An uploaded file (photo, document, video, presentation, etc.)
- A text evidence (written description) of what you accomplished
Together, the written reflection and supporting materials help verify your experience and the skills you’ve demonstrated.
Add Evidence
- Click on the Evidence icon (
) in the navigation menu. - Click on the red Add Evidence button in the top-right corner of the screen.
- At the top, you’ll see three cards for choosing your evidence type. Select the option that matches how you want to submit your evidence:
- Text (
) – Type your evidence directly into the form - Website (
) – Paste a link to a webpage, project, or other online content - Upload (
) – Upload a file, such as a photo, document, video, or presentation
- Text (
- Fill out the required fields based on your selected evidence type. (Some fields may look slightly different depending on your choice.)
- Click the red Next button in the bottom-right corner of the screen.
- Fill out the experience feedback fields as prompted.
- Click the red Next button to continue.
- Review your evidence submission. To make changes, click on the Edit Icon (
) next to each section. - When everything looks correct, click the red Submit button in the bottom-right of the screen.
Add Evidence for an Assignment
- Click on the Assignments icon (
) in the navigation menu. - Select the assignment by clicking its card.
- Scroll down to the Performance Tasks or Reflection Tasks section in the assignment overview.These tasks were created by the person who designed the activity—like your teacher. Read each task carefully to understand what’s expected before you start your evidence submission.
- Performance Tasks (sometimes called Custom Tasks) are unique tasks with specific instructions. You might see several different tasks, each asking for something different.
- Reflection Tasks use the same prompt and skills across all entries. These are often used for open-ended or ongoing experiences, like work-based learning or volunteer work.
- Click the Let’s Start button next to the task you want to complete.
- Follow the step-by-step prompts of the evidence form to submit your evidence.

- Fill out the required fields. These may vary based on how your teacher set up the assignment.
Hint: Click on a Skill hexagon to view more details about that specific skill - When you’re finished, click the red Submit button.

Note: Certain fields may be locked or grayed out—these were set by your teacher and cannot be changed. - Click the red Next button in the bottom-right corner of the screen.

- Review your evidence submission. To make changes, click on the Edit Icon (
) next to each section. - When everything looks correct, click the red Submit button.

Add Evidence From a Seal, Opportunity, Skill, or Dashboard
From a Seal or Opportunity:
- Click on the Opportunities icon (
) in the navigation menu. - Choose a Seal or Opportunity and click into it to view the details.
- Click on a Skills hexagon.
- Click the red Add Evidence button. You’ll be taken to an evidence form with the skill already selected.
- Complete the evidence form. Continue from Step 3 in the Add Evidence instructions above.
From a Skill:
- Click on the Opportunities icon (
) in the navigation menu. - Toggle to the Skills section by using the red and white button in the upper-right of the page.
- Choose a Skill and click the hexagon to view the details.
- Click the red Add Evidence button. You’ll be taken to an evidence form with the skill already selected.
- Complete the evidence form. Continue from Step 3 in the Add Evidence instructions above.
From the Dashboard:
- Click on the Dashboard icon (
) in the navigation menu. - You can create evidence from two dashboard sections: Work Based Learning and Community Service. Click Add Evidence in either section to open the evidence form.
- If you use Work Based Learning, it will be pre-filled with work-based learning selected.
- If you use Community Service, it will be pre-filled with community service selected.
- Complete the evidence form. Continue from Step 3 in the Add Evidence instructions above.

View Evidence and Evaluations
- Click on the Evidence icon (
) in the navigation menu. - From this page, you can view all your evidence submissions.
(Switch between list and card views using the red and white toggle button in the upper-right corner. The List icon (
) represents the list view and and the Grid icon (
) represents the card view. Use the filters to narrow results by status (Submitted or Evaluated), assignment, skills, community service type, and more. You can also use the sort feature in the list view by clicking the arrows (
) next to column headers to organize your evidence by title, submitted date, work-based learning, or other details. The arrows appear when you hover over the column headers.) - To open a specific piece of evidence, click its row in list view (
) or click the card in card view (
)
(In addition to your evidence, if it has already been evaluated, you’ll see your evaluation results—this includes which skills you passed and any feedback from your evaluator.)
Edit Evidence
- Click on the Evidence icon (
) in the navigation menu. - From this page, you can view all your evidence submissions.

Note: You cannot edit evidence that has already been evaluated. To view only the evidence you can still edit, set the Status filter to Submitted. - In the evidence form, click on the
Edit Evidence button next to each section to make changes. When you’ve made the changes, click
Save Changes. - When you’re finished, click the red Save button at the bottom-right of the screen.
